I, the parent or guardian named in this application, authorize the ministry staff of Family Christian Centre to sign a consent for medical treatment and to authorize any physician or hospital to provide medical assessment, treatment or procedures for the child named above. I undertake and agree to indemnify and hold blameless the ministry staff and volunteers, Family Christian Centre, its pastors, and Pastor’s Council from and against any loss, damage or injury suffered by the child as a result of being part of the programs of Family Christian Centre as well as of any medical treatment authorized by the supervising individuals representing the church. Family Christian Centre will be hosting Live It Out Discipleship Camp for children Gr.1 (as of Fall 2025) - Gr.4
Live It Out is not just a theme; it’s a call to action. Taking place 10am to 12pm(noon) each day.
It encourages children to actively live out their faith through their words and interactions with others.

Children: Grade 1 (as of Fall 2025) - Grade 4
$50.00 per child
Third child (and any additional in the household) $35 each.

VOLUNTEERS
Camp doesn't happen without our volunteers, please check off the box if you yourself would like to volunteer!!
Volunteer Discount - $15 off per child providing you volunteer for the entire Discipleship Camp.
